What Is Home Health Care?
Home health care is a wide range of health care services that can be given in your home for an illness or injury. Home health care is usually less expensive, more convenient, and just as effective as care you get in a hospital or skilled nursing facility (SNF).
The goal of home health care is to treat an illness or injury. Home health care helps you get better, regain your independence, and become as self-sufficient as possible.

Skilled Nursing
- Skilled assessment, observation, and instruction of the disease process
- Educates on medications, diet, safety precautions, emergency measures, and available community resources
- Coordinates and involves other services such as physicians, therapists, medical social workers, aides, and dietitians during the entire treatment plan
- Wound care assessment, instructions, and treatments
- Ostomy care and instructions
Physical Therapy
- Therapeutic home exercise program
- Transfer, gait/balance, and mobility training
- Proper safety use of assistive devices (eg. Wheelchair, Walker, Cane)
- Prosthetic training
Occupational Therapy
- Evaluates the patient’s functional level
- Guides the patients in the performance of therapeutic, creative, and self-care activities.
- Helps in the adaptation of the living environment
Speech Therapy
- Evaluates speech and swallowing abilities and provide an approved rehabilitation program for speech and swallowing impairment
Medical Social Worker
- Assists the physicians and the team in understanding the significance of social and emotional factors related to the patient’s health problems
- Assists patients in obtaining and utilizing available community resources
Home Health Aide
- Assists and instructs patient/caregivers/family in performing personal care and their activities of daily living (e.g., giving baths, catheter care, ROM exercises, transferring, and assistance with ambulation)
Other
- We can arrange dieticians, podiatrist, laboratory and diagnostic services, medical equipment, prescribed medication delivery, and home medical doctors.
